Forms & Insurance

Centurion Physical Therapy will contact your insurance company prior to your first appointment to obtain your benefit information including coverage and deductibles. If you do not have insurance coverage, we do offer self-pay rates.

Payment is due at the time services are rendered and as a courtesy, we will submit an insurance claim on your behalf. By request, we can keep your credit card on file.

In order to optimize the time during your initial evaluation, we ask that you complete the New Patient Registration form in advance. Many insurance companies require additional paperwork; we can send you the appropriate forms after our initial conversation.

All forms can be completed and faxed or emailed to us ahead of time or brought with you to your first appointment.
